Out of Network HMO / IPA Members:

As of November 1st 2010, Reliant will require an $85.00 deposit for patients to be seen that are members of any HMO listed below.  As a courtesy, Reliant will bill your insurance carrier for the visit and attempt to be paid.  If we are paid, you will be refunded the $85.00.  At your request,  we can attempt to obtain an authorization for the visit.
